Donation after circulatory determination of death (DCDD) is an accepted practice in the United States, but heart procurement under these circumstances has been debated. Although the practice is experiencing a resurgence due to the recently completed trials using ex vivo perfusion systems, interest in thoracoabdominal normothermic regional perfusion (TA-NRP), wherein the organs are reanimated in situ prior to procurement, has raised many ethical questions. We outline practical, ethical, and equity considerations to ensure transplant programs make well-informed decisions about TA-NRP. We (...) present a multidisciplinary analysis of the relevant ethical issues arising from DCDD-NRP heart procurement, including application of the Dead Donor Rule and the Uniform Definition of Death Act, and provide recommendations to facilitate ethical analysis and input from all interested parties. Polyphenic differences between individuals arise not through differences at the genome level but as a result of specific cues received during development. Polyphenisms often involve entire suites of characters, as shown dramatically by the polyphenic castes found in many social insect colonies. An understanding of the genetic architecture behind polyphenisms provides a novel means of studying the interplay between genomes, gene expression and phenotypes. Here we discuss polyphenisms and molecular genetic tools now available to unravel their developmental bases in insects. (...) Medical ghostwriting is the practice in which pharmaceutical companies engage an outside writer to draft a manuscript submitted for publication in the names of “honorary authors,” typically academic key opinion leaders. Using newly-posted documents from paroxetine litigation, we show how the use of ghostwriters and key opinion leaders contributed to the publication of a medical journal article containing manipulated outcome data to favor the proprietary medication. The article was ghostwritten and managed by SmithKline Beecham, now GlaxoSmithKline (GSK) and Scientific Therapeutics (...) Information, Inc. without acknowledging their contribution in the published article. The named authors with financial ties to GSK, had little or no direct involvement in the paroxetine 352 bipolar trial results and most had not reviewed any of the manuscript drafts. The manuscript was originally rejected by peer review; however, its ultimate acceptance to the American Journal of Psychiatry was facilitated by the journal editor who also had financial ties to GSK. Thus, GSK was able to take an under-powered and non-informative trial with negative results and present it as a positive marketing vehicle for off-label promotion of paroxetine for bipolar depression. In addition to the commercial spin of paroxetine efficacy, important protocol-designated safety data were unreported that may have shown paroxetine to produce potentially harmful adverse events. Media exposure can shape audience perceptions surrounding novel innovations, such as artificial intelligence (AI), and could influence whether they share information about AI with others online. This study examines the indirect association between exposure to AI in the media and information sharing about AI online. We surveyed 567 US citizens aged 18 and older in November 2020, several months after the release of Open AI’s transformative GPT-3 model. Results suggest that AI media exposure was related to online information sharing through (...) psychological proximity to the impacts of AI and positive AI performance expectancy in serial mediation. This positive indirect association became stronger the more an individual perceived society to be changing due to new technology. Results imply that public exposure to AI in the media could significantly impact public understanding of AI, and prompt further information sharing online. (shrink)

From the obtained characterization it follows that all countably-categorical elementary theories of Boolean algebras with distinguished ideals are finite-axiomatizable, decidable and, consequently, their countable models are strongly constructivizable.

Insights from First Amendment considerations and from developmental psychology are utilized in suggesting that whatever value codes of ethics may hold for the mass media, they represent serious difficulties in inculcating substantial ethical values in individual journalists and in the profession as a whole. Evidence from developmental psychology suggests that codes are probably of some limited value to the neophyte working in the media. Codes also help assure non?journalists that the industry really is concerned about ethics. However, codes probably should (...) be relegated to a framed wall hanging for any journalists who have advanced beyond their internships. Confusion reigns because codes are often founded on moralistic rather than moral?philosophical bases, and there is a blurring between general precepts and specific practices covered in codes. As individual professionals mature intellectually and ethically, they should transcend socially?approved conventions codified by ?regulators,?; and begin to become social catalysts in their own rights, according to this essay. The current study uses a sensemaking model and thinking strategies identified in earlier research to examine ethical decision making. Using a sample of 163 undergraduates, a low-fidelity simulation approach is used to study the effects personal involvement (in causing the problem and personal involvement in experiencing the outcomes of the problem) could have on the use of cognitive reasoning strategies that have been shown to promote ethical decision making. A mediated model is presented which suggests that environmental factors influence reasoning (...) strategies, reasoning strategies influence sensemaking, and sensemaking in turn influences ethical decision making. Findings were mixed but generally supported the hypothesized model. It is interesting to note that framing the outcomes of ethically charged situations in terms of more global organizational outcomes rather than personal outcomes was found to promote the use of pro-ethical cognitive reasoning strategies. Loneliness in medicine is a serious problem not just for patients, for whom illness is intrinsically isolating, but also for physicians in the contemporary condition of medicine. We explore this problem by investigating the ideal physician-patient relationship, whose analogy with friendship has held enduring normative appeal. Drawing from Talbot Brewer and Nir Ben-Moshe, we argue that this appeal lies in a dynamic form of companionship incompatible with static models of friendship-like physician-patient relationships: a mutual refinement of embodied virtue that draws (...) both persons together. The ideal physician-patient relationship has a dialectical character that fosters each member's improvement of phenomenologically recognizing and embodying moral virtues. A key component of this dynamic is a commitment to the common goal of the patient's health, realized through joint interactivities and conversations over time. The physician's presence to the patient's suffering—understood best as an alienating phenomenological condition for the patient—orients and discloses possibilities for virtuous caregiving by structuring the meanings of the goals, conversations, and joint narrative constitutive of their relationship. Presence to suffering, paradoxically, is perhaps an important prerequisite for this dynamic partnership. These activities dialectically build an interpretive horizon of understanding through which moral goods and character refinement—in and for the other—may become revealed for both persons in their shared being-in-the-world. This analysis of suffering, mood, and revealing of (possible) moral goods has implications for addressing the modern problem of loneliness for patients and physicians, who are increasingly inhibited from building flourishing relationships with each other. The consensus recommendations by Salter and colleagues (2023) regarding pediatric decision-making intentionally omitted adolescents due to the additional complexity their evolving autonomy presented. Using two case studies, one focused on truth-telling and disclosure and one focused on treatment refusal, this article examines medical decision-making with and for adolescents in the context of the six consensus recommendations. It concludes that the consensus recommendations could reasonably apply to older children.

Despite the lack of invariance problem (the many‐to‐many mapping between acoustics and percepts), human listeners experience phonetic constancy and typically perceive what a speaker intends. Most models of human speech recognition (HSR) have side‐stepped this problem, working with abstract, idealized inputs and deferring the challenge of working with real speech. In contrast, carefully engineered deep learning networks allow robust, real‐world automatic speech recognition (ASR). However, the complexities of deep learning architectures and training regimens make it difficult to use them to (...) provide direct insights into mechanisms that may support HSR. In this brief article, we report preliminary results from a two‐layer network that borrows one element from ASR, long short‐term memory nodes, which provide dynamic memory for a range of temporal spans. This allows the model to learn to map real speech from multiple talkers to semantic targets with high accuracy, with human‐like timecourse of lexical access and phonological competition. Internal representations emerge that resemble phonetically organized responses in human superior temporal gyrus, suggesting that the model develops a distributed phonological code despite no explicit training on phonetic or phonemic targets. The ability to work with real speech is a major advance for cognitive models of HSR. (shrink)

Functional diversity holds the promise of understanding ecosystems in ways unattainable by taxonomic diversity studies. Underlying this promise is the intuition that investigating the diversity of what organisms actually do—i.e. their functional traits—within ecosystems will generate more reliable insights into the ways these ecosystems behave, compared to considering only species diversity. But this promise also rests on several conceptual and methodological—i.e. epistemic—assumptions that cut across various theories and domains of ecology. These assumptions should be clearly addressed, notably for the sake (...) of an effective comparison and integration across domains, and for assessing whether or not to use functional diversity approaches for developing ecological management strategies. The objective of this contribution is to identify and critically analyze the most salient of these assumptions. To this aim, we provide an “epistemic roadmap” that pinpoints these assumptions along a set of historical, conceptual, empirical, theoretical, and normative dimensions. (shrink)

We estimate that 208,000 deep brain stimulation (DBS) devices have been implanted to address neurological and neuropsychiatric disorders worldwide. DBS Think Tank presenters pooled data and determined that DBS expanded in its scope and has been applied to multiple brain disorders in an effort to modulate neural circuitry. The DBS Think Tank was founded in 2012 providing a space where clinicians, engineers, researchers from industry and academia discuss current and emerging DBS technologies and logistical and ethical issues facing the field. (...) The emphasis is on cutting edge research and collaboration aimed to advance the DBS field. The Eighth Annual DBS Think Tank was held virtually on September 1 and 2, 2020 (Zoom Video Communications) due to restrictions related to the COVID-19 pandemic. The meeting focused on advances in: (1) optogenetics as a tool for comprehending neurobiology of diseases and on optogenetically-inspired DBS, (2) cutting edge of emerging DBS technologies, (3) ethical issues affecting DBS research and access to care, (4) neuromodulatory approaches for depression, (5) advancing novel hardware, software and imaging methodologies, (6) use of neurophysiological signals in adaptive neurostimulation, and (7) use of more advanced technologies to improve DBS clinical outcomes. There were 178 attendees who participated in a DBS Think Tank survey, which revealed the expansion of DBS into several indications such as obesity, post-traumatic stress disorder, addiction and Alzheimer’s disease. The U.S. Food and Drug Administration's rationale for supporting the development and approval of BiDil for heart failure specifically in black patients was based on under-powered, post hoc subgroup analyses of two relatively old trials , which were further complicated by substantial covariate imbalances between racial groups. Indeed, the only statistically significant difference observed between black and white patients was found without any adjustment for potential confounders in samples that were unlikely to have been adequately randomized. Meanwhile, because the accepted (...) baseline therapy for heart failure has substantially improved since these trials took place, their results cannot be combined with data from the more recent trial amongst black patients alone. There is therefore little scientific evidence to support the approval of BiDil only for use in black patients, and the FDA's rationale fails to consider the ethical consequences of recognizing racial categories as valid markers of innate biological difference, and permitting the development of group-specific therapies that are subject to commercial incentives rather than scientific evidence or therapeutic imperatives. This paper reviews the limitations in the scientific evidence used to support the approval of BiDil only for use in black patients; calls for further analysis of the V-HeFT I and II data which might clarify whether responses to H-I vary by race; and evaluates the consequences of commercial incentives to develop racialized medicines. We recommend that the FDA revise the procedures they use to examine applications for race-based therapies to ensure that these are based on robust scientific claims and do not undermine the aims of the 1992 Revitalization Act. The conflict of religions during the Christianization of the Greco-Roman aristocracy in Late Antiquity is typified by Synesius, an old-fashioned pagan Neoplatonist who studied under Hypatia at Alexandria, yet who in A.D. 410 became the Christian bishop of Ptolemais in Libya. Before accepting, however, he openly stated his objections to certain Christian dogmas. Was he a Christian or a "baptized Neoplatonist"? The generation of Synesius saw the rapid decline of paganism. Furthermore, the Constantinople he visited was a Greek-Christian Rome whose (...) elites were classically educated. He returned home an ally of the city's Orthodox Christians. He tried to reconcile Neoplatonism with Christianity, but a study of his works demonstrates that he was only partially successful. Synesius is important for our understanding of the old aristocracy in Late Antiquity. His becoming a bishop completes the picture in which we finally see the ancient world transforming itself into the medieval world. The life of Synesius, one man of Late Antiquity, may be viewed as both the recapitulation and anticipation of all the major themes of Classical and Late Antiquity. Manuscript authority, in the person of the so-called ‘corrector’, supports the attribution. Yet, at first sight at least, the attribution of this poem to Crinagoras raises something of a problem. It does so because the poem evidently relates to what seems to be a contemporary marriage linking the royal families of Egypt and Libya respectively: if the author (...) is indeed Crinagoras, what marriage can this be? Christopher D. Stone has claimed that natural objects can and should have rights. I accept Stone’s premise that the possession of rights is tied to the possession of interests; however, I argue that the concept of a natural object needs a more careful analysis than is given by Stone. Not everything that Stone calls a natural object is an object “naturally.” Some must be taken as artificial rather than as natural. Thistype of object cannot be said to have intrinsic interests (...) and hence cannot be given rights that can protect “its own sake”-which is the sort of right that Stone focuses on. Further, there are other sorts of natural objects which, although they are objects “naturally,” cannot meaningfully be said to have intrinsic interests, and thus cannot have the sorts of rights that Stone is concerned with. Finally, there are other sorts of natural objects which are objects naturally and which have intrinsic interests, but which are not proper candidates for the possession of rights. The prerequisites for being “owed” rights are the possession of intrinsic interests and the capability to suffer when those interests are interfered with or denied or threatened. (shrink)

In the iconic measurements of atomic spin-1/2 or photon polarization, one employs two separate noninteracting detectors. Each detector is binary, registering the presence or absence of the atom or the photon. For measurements on a d-state particle, we recast the standard von Neumann measurement formalism by replacing the familiar pointer variable with an array of such detectors, one for each of the d possible outcomes. We show that the unitary dynamics of the pre-measurement process restricts the detector outputs to the (...) subspace of single outcomes, so that the pointer emerges from the apparatus. We propose a physical extension of this apparatus which replaces each detector with an ancilla qubit coupled to a readout device. This explicitly separates the pointer into distinct quantum and (effectively) classical parts, and delays the quantum to classical transition. As a result, one not only recovers the collapse scenario of an ordinary apparatus, but one can also observe a superposition of the quantum pointer states. (shrink)

Beginning with Fanon’s challenge to the universality of the project of ontology, this paper considers whether and how Merleau-Ponty’s early and late thinking may yield a response. From the outset, Merleau-Ponty’s appeal to the materiality of the body is intended as a limit on the scope of ontology. As I argue, however, Merleau-Ponty’s early concept of ‘one’s own body’ suggests an “ontological equality” that would be shared among all embodied beings; implicitly, this early approach risks reinforcing Fanon’s concern that ontology (...) is indifferent to embodied experiences of racial exclusion and oppression. Merleau-Ponty’s later ontology of the flesh, by contrast, entails a more radically differential structure of the body that troubles the notion of equality in principle, suggesting an ontology that could be more attentive to the fundamental grounds of systemic oppression.Cet article examine si, et de quelle manière, la réflexion de Merleau-Ponty peut répondre au défi lancé par Fanon à l’universalité du projet ontologique. This cross-sectional study, which included men aged 20–40 years, aimed to determine the relationships among type D personality, depressive symptoms and lower urinary tract symptoms in young men. An internet-based questionnaire was administered, and General demographics, International Prostate Symptom Scores, Type D Scale-14 scores, and Depression and Somatic Symptom Scale scores were analyzed. A total of 3,127 men were included; of these, 762 reported moderate/severe lower urinary tract symptoms, and 1,565 met the criteria for type D personality. Men with type (...) D personality had significantly higher body mass index and total and sub-scores for the International Prostate Symptom Score and Depression and Somatic Symptom Scale. Furthermore, the type D personality group had a higher prevalence of lower urinary tract symptoms, particularly voiding symptoms. Univariate analysis revealed that all parameters, except for body mass index, were significant predictors of moderate/severe lower urinary tract symptoms. Multivariate analysis showed that age >30 years, type D personality, and depressive and somatic Depression and Somatic Symptom Scale sub-scores were independent predictors of moderate/severe lower urinary tract symptoms. Regarding Type D Scale-14 subscales, social inhibition, rather than negative affectivity, impacted moderate/severe lower urinary tract symptoms. Mediation analysis revealed that depressive symptoms mediated the relationship between type D personality and lower urinary tract symptoms. This study established correlations between type D personality, depressive symptoms, and lower urinary tract symptoms. As previous studies suggested that patients with type D personality are less likely to consult and adhere to treatment, and are at higher risk for depression, urologists should therefore actively recognize patients with TDP. Recently, there has been a rise in pessimism concerning what theoretical ecology can offer conservation biologists in the formation of reasonable environmental policies. In this paper, I look at one of the pessimistic arguments offered by Kristin Shrader-Frechette and E. D. McCoy (1993, 1994)--the argument from conceptual imprecision. I suggest that their argument rests on an inadequate account of the concepts of ecological stability and that there has been conceptual progress with respect to complexity-stability hypotheses. Many are reading books about Buddhism, and some are also meditating, participating in Buddhist retreats, and studying under Buddhist teachers. As they do so, they approach what might be called "dual religious belonging."The phrase itself can (...) suggest at least three metaphors. We can imagine them (1) as people crossing a bridge into the world of Buddhism and who then return to Christianity with fresh insights; or (2) as people with two intravenous tubes in their arms, one providing fluid from a Buddhist lineage and one providing fluid from a Christian lineage, for the sake of a more complete life; or, shifting to a more organic metaphor, (3) as people with primary roots in Christian soil but with secondary roots in Buddhist soil, who receive anchorage and sustenance from both kinds of soils.Shortly I will draw upon the third metaphor to suggest the desirability of a "taproot" as opposed to a "fibrous" approach for such double belonging, at least in its initial stages. First, a word is in order about the philosophical backdrop for my approach, which draws upon the philosophy of Alfred North Whitehead and its theological counterpart, process theology. Toward this end, I will draw upon the first metaphor: the bridge. A Bridge for Double Belonging Process theology emerged in the 1930s at the University of Chicago and has since been used by many Christians and some Buddhists, mainly but not exclusively Pure Land Buddhists, to interpret their respective religious perspectives. I myself have used this theology in several books to try to show how Christians can draw deeply from the wells of Buddhist insight and practice in ways that enrich and deepen Christian life.Process theology draws upon the "philosophy of organism" developed by the late philosopher and mathematician Alfred North Whitehead. Whitehead's philosophy is unique in that it has deep similarities with numerous Buddhist points of view even as it also offers a rich way of interpreting and appreciating core Christian insights. [End Page 67] It is no accident that process theologians such as John Cobb suggest that process theology is a way of seeing things—a theological bridge—that can help a Christian cross over into Buddhism, learn from it in deep ways, and return with fresh insights that enrich a walk with Christ; and that can also help Buddhists cross over into Christianity, learn from it in deep ways, and return with fresh insights for practicing the dharma.What, then, are the core teachings of process theology? If we imagine process theology as a wooden bridge, here would be ten of its planks:There are many worlds beyond words, which can be known in many ways, including prayer and meditation, empathy and imagination, intuition and mindful perception.Human life consists of a series of moments of experience, each of which includes, and is dependent on, the whole of the universe.The universe is an unfolding process that is never the same at two instants.Humans and other living beings have no substantial or permanent self that separates them from the surrounding world.Ultimate reality is a nonstatic and nondualistic Emptiness of which all things are manifestations: with Emptiness referring to (a) the emptiness of words and concepts as adequate descriptions of things as they are, (b) the sheer presence of things as they are in their suchness, (c) the absence of a self-contained substantiality or "own being" within things, (d) the sheer interconnectedness of all things, (e) the pure becoming of all things.There is a womblike presence—God—who shares in the joys and sufferings of each living being, and who is herself an example of no-self, interdependence, pure becoming, of the Emptiness described above.God is present within each living being an in-dwelling lure toward wisdom and compassion.God's presence is also found between humans, when they live in community with one another in a...
